The Significance of P duration and P-R Segment of the Electrocardiogram

Abstract
Macruz described a new method to differentiate ECG findings between the left and right atrial enlargement by using a ratio of P duration/ P-R Segment.
This principle appears to be derived only from the relation between anatomical changes of a heart and ECG. Authors evaluated the Macruz¢¥ s method on 42 normal Korean males before and after the Master¢¥ s Single and Double two-step test, 23 cases of the First grade A-V block, 1 case of W-P-W Syndrome, 16 cases of Mitral stenosis without conduction defect and 18 cases of Cor pulmonale.
The P/P-R ratio of 42 normal Korean males was 1.29(1.2~1.66) except two cases with P-R interval 0.12 seconds, of which P/P-R ratios were 2.0 In 23 cases of the first Grade A-V Block the ratio was 0.95 (O. 5~1.5) which was indicative of a right atrial enlargement according to the Macruz¢¥-s criteria. In a case of-W-P -W Syndrome the P/P-R ratio was impossible because the P-R segment was zero.
In 16 cases of mitral stenosis the ratio was 1.8(1.66~3.0)which was indicative of a left atrial enlargement according to the Macruz¢¥ s criteria. However in 18 cases of Cor pulmonale the ratio was 1.5(1.0-2.0) and this data was inconclusive.
Above data showed that in cases of marked acceleration of the A-V conduction or the 1st grade A-V block, the Macruz¢¥ s criteria was not tenable, however a mild and moderade exercise or acceleration of heartrate did :not change the P/P-R ratio significantly. Authors concluded that the P/P-R ratio depended not only on the anatomical but also on functional changes of the atrium such as conduction disturbance. In cases of the P-R interval between 0.12 seconds and -0.20 seconds the Macruz¢¥ s criteria was tenable and his method was considered to be useful for ECG analysis.
